New Patient Registration
If you are moving into our practice area and would like to register with us, please check our Boundary Map to see if your new address falls within our boundary; if you live outside this area we may not be able to register you here.
Catchment Area
If you live outside this area, your registration may be rejected.
Register Online
As it often takes some time to records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire.
Although not mandatory, it can help the Practice if you provide photo ID and proof of address with your online registration form.
Medical treatment is available from the date of registration. Please contact reception for further information.
Please complete a form for each member of your household who wish to register.
If you prefer not to register online please complete the following steps:
- Please check our map (above) to ensure you live in the catchment area
- Download, print and complete the NHS GP patient registration form, also available from reception.
- Return your completed registration forms to reception or post to us
- If you don’t have an access to a printer and would like us to post a registration form to you please give us a call on 020 8520 2625

Your Named GP
The surgery allocates all patients with a named GP who will have overall responsibility for the care and support that our surgery provides to them at the point of registration.
We know that some patients will wish their named GP to be a particular doctor. Where a patient expresses a preference as to which doctor they have been assigned, the practice will make reasonable efforts to accommodate this request, but we would like to reassure patients that this system does not prevent you from seeing any GP in the practice as you currently do. Your named GP will not be available at all times.
If you are unsure of who your named GP is, please ask the Reception Team when you next contact the practice.
